Referrals

Referrals to the Lake County Wraparound Program can be made by services providers, community partners, faith based institutions, mentors, coaches, educators, parents, and friends of the youth and families, and by youth/families may self –refer.

Once a referral is made, a Family Care Coordinator will contact the family/youth to set up an initial orientation meeting to help them learn about the process and answer any questions to help them make an informed decision about participating in Wraparound services.

If you are an individual not connected to an organization and want to make a referral you can contact us directly without a referral form.
