High Fidelity Wraparound

Through our High Fidelity Wraparound services, youth and families work with a Wraparound Care Coordinator for 6 months to a year or longer if necessary. 

In this process the youth/family work with their care coordinator to create a team of professionals as well as their own natural supports to develop a long term vision for their future. The family and the team meet regularly over the course of the process in Child and Family Team Meetings to create a care plan with short term and long term goals to reach their vision. Through this process families learn to identify and plan for their own needs, stabilize and prevent crisis, resolve conflict and restore relationships. The model follows four phases and ten guiding principles. 

Transitional Living Program

Youth ages 16 through 21 who are or are at risk of becoming homeless. This program supports youth with long-term (12-18 months) services to create and maintain housing stability. 

Individual Service Support Team (ISST)

Through our Individual Services Support Team (ISST), youth and families work with a Care Coordinator for a short period of time of 1- 3 months.  This process focuses on short term intervention.

McKinney Vento

Care Coordination/Case Management for children and youth enrolled in Preschool through 12th grade and their caregivers or unaccompanied minors experiencing homelessness .  The Lake County Wraparound Program provides care coordination and individual case management to support the youth and families in meeting their basic needs as well as supporting in transitioning into stable housing and meeting their educational and other needs.

Basic Center Program

Care coordination and individual case management for unaccompanied youth between the ages of 12 through 17 years of age who are runaways or homeless or are at-risk of becoming  runaway or homeless.


 

WHO QUALIFIES:

Children/youth struggling with repeated absences and failing grades at school or refusal to attend school

Children and youth with repeated discipline referrals at school or suspensions

Children and youth expelled or at risk of getting expelled from school

Children and youth in youth in alternative education

Children and youth engaging in substance use

Children and youth struggling with discipline at home and/or having major conflict with their care givers and family members

Children and youth self-harming

Children and youth experiencing difficulties/ crisis from a major loss or change in their life such as death, divorce, relocation, or a traumatic event

Children and youth suffering from a mental health condition.

Children and youth having trouble with the law

Children and youth who have been placed out of their home for any reason

Children and youth who are homeless or at risk of becoming homeless

Children and youth in foster care or involved with child protective services.
